About the lesson
The main goal of social prescribing is to address the needs of patients, in a person-centered way, with a community-led approach, so that they feel empowered and have more control over their own health and wellbeing.
Social prescribing gives GPs, and some other healthcare professionals, a way to refer patients with social, emotional and practical needs to sources of support within their communities.
